PARENTAL CONSENT FORM

Users who are aged under 12 must be accompanied by a adult when visiting a Belong Arena. 

Players who are aged to 7 -9 can be consented to play ESRB games rated 10+ and Teen by completing the consent form by a parent or guardian.

Players who are aged to 10 -12 can be consented to play ESRB games rated  Teen by completing the consent form by a parent or guardian.   

Players who are aged to 13-17 can be consented to play ESRB games rated mature by completing the consent form by a parent or guardian.   

You cannot consent to play ESRB games rated 18+ .Players must be 18 and over.
